If you are a new patient to Dental Design Innovations, once you have scheduled an appointment and are in our system you may register and complete all required forms on our Patient Portal (link at the top of the page).
If you prefer to complete these forms at our office instead, please arrive 20 minutes before your appointment time to do so.

Insurance
We are not a participating provider with any insurance plans, but we will file your insurance claim for your visit so that you can be reimbursed the maximum benefit from your particular dental plan.
Most patients receive their payments within 2-3 weeks. We are happy to assist in contacting your insurance company if you fail to receive payment in a timely manner.
Payments Accepted
For your convenience, we accept Visa, MasterCard, American Express, Discover, personal checks, cash, and CareCredit (a healthcare credit card that offers interest-free financing to qualified applicants) for payment of services rendered. Payment is expected at the time services are performed.
If you carry dental insurance and have a treatment plan consisting of major restorations, we can request a pre-treatment estimation of benefits with your plan.
